T/C Rod nut/washer/bushing fell off

Well my T/C Rod nut/washer/bushing fell off. Well I can say that's an easy way to make a turning brake. Anyways should I do to make sure that this doesn't happen again ? so far I'm missing the nut washer and fairly new poly bushing. Just to confirm I should have the poly bushing on the FRONT of the the car on my S130 ?

Okay I went to the junk yard and picked up three nuts one washer and one retainer washer (?) anyways I put the new bushing in and was able to put one nut on all the way and the other one half way. But on the other side that did not fall off there is only maybe one or two threads exposed and I can't really tighten it any more. What should I do now that one is really tight and one is really but with a lot of thread engagement
